Please send us the
following:
a. An ECS application
b. Developmental analysis (e.g., an IQ
test and other standardization tests)
c. Diagnosis
d. If
your child has received services from another behavioral consultant, please
provide a program update that includes:
* name of all mastered programs with each SD mastered
* all current programs and current SDs
* the name and phone number of your current consultant
* date of initial workshop
Once ECS has received all of your information, your consultant will call you to schedule an initial workshop.
If you have qualified for
the PDD Waiver through DDSN, you will not be charged for the hours they have
agreed to cover. The waiver does not
cover educational programs. This means
that shadow aides and observation of those aides by your consultant are not
covered costs. You may want to consider
looking into a non-educational program if this is a concern for you. The waiver only covers six hours a month of
consulting. Most families will need more
than six hours of consulting a month.
Anything over the initial six hours will be billed to the family at $80
per hour. Call the ECS office for more
details on the waiver.
